Lapsed Credential Policy

 

A credential can only be expired for a period of 1 year. After 90 days the credential is considered lapsed, deemed inactive and has to be reinstated. The credential is eligible for reinstatement for a period of 1 year from the expiration date.  After that time, you must retest and meet the current exam eligibility criteria to reinstate.

Verification of the credential will be reported as lapsed to third parties once expired after 90 days.  The credential can be renewed during the one year reinstatement period without re-testing. The credential will NOT be extended. 

Reinstatement Requirements

Within 1 year of Expiration

  • Practice Credentials
    • Meet Regular Renewal Requirements
    • Additional 12-24 CE hours based on that amount of time credential lapsed
    • Submit non-refundable and non-transferable Renewal Application and Late Fee ($350)
  • Specialty Certificates
    • Meet Regular Renewal Requirements
    • Additional 2-4 CE hours based on that amount of time credential lapsed
    • Submit non-refundable and non-transferable Renewal Application and Late Fee ($250)

After 1 year of Expiration

  • Practice Credentials
    • Meet Regular Renewal Requirements
    • Additional 12-24 CE hours based on that amount of time credential lapsed
    • Submit non-refundable and non-transferable Renewal Application and Late Fee ($350)
    • Re-Test
  • Specialty Certificates
    • Meet Regular Renewal Requirements
    • Additional 2-4 CE hours based on that amount of time credential lapsed
    • Submit non-refundable and non-transferable Renewal Application and Late Fee ($250)
    • Re-Test
